CIHR (Canadian Institutes of Health Research), ZonMw (de Nederlandse organisatie voor
gezondheidsonderzoek en zorginnovatie) en het Reumafonds kondigen met trots een
samenwerkingsinitiatief aan: het Canadees-Nederlandse netwerk voor Personalised Medicine bij
immuungemedieerde inflammatoire musculoskeletale aandoeningen.
Dit samenwerkingsverband richt zich op de ontwikkeling van een gepersonaliseerde
farmacotherapeutische aanpak bij de behandeling van immuungemedieerde inflammatoire
musculoskeletale aandoeningen (IMIMD). Hierbij wordt gebruikgemaakt van het onderzoekspotentieel van
de patiëntencohorten in Canada en Nederland.
Samenwerking veelbelovend voor ontwikkeling gepersonaliseerde behandelingen
Het groeiende arsenaal aan doelgerichte farmacotherapieën is een revolutionaire ontwikkeling op het
gebied van de behandeling van een breed spectrum aan IMIMD. Er bestaat internationale consensus over
het feit dat aan deze ontwikkeling op farmacotherapeutisch gebied een aanzienlijk kostenplaatje hangt.
Voor deze biotechnologische behandelmethoden lopen de kosten op tot enkele tienduizenden
euro’s/dollars per persoon per jaar, ongeacht welke behandeling wordt gekozen.
Ook het spectrum van inflammatoire aandoeningen waarvoor deze behandelingen worden gebruikt breidt
zich uit. In sommige gevallen leidt dit tot onverwachte resultaten, zoals bleek toen recent werd aangetoond
dat dodelijke vormen van vasculitis reageren op de depletie van B-lymfocyten, een behandelmethode die
wordt toegepast bij reumatoïde artritis en lymfoom. Met deze farmacotherapeutische ontwikkeling gaat een
verschuiving in het onderzoek naar de biomarkers gepaard, die veelbelovend is voor gepersonaliseerde
behandelingen van complexe inflammatoire aandoeningen.
De kracht van het samenbrengen van patiëntencohorten
Een belangrijke factor voor het personaliseren van farmacotherapeutische behandelingen bij complexe
inflammatoire aandoeningen zijn grote, goed gekarakteriseerde patiëntencohorten, zowel op nationaal als
internationaal niveau. Idealiter zouden deze cohorten in netwerken georganiseerd moeten zijn, om zo te
kunnen profiteren van de kracht van meerdere, zeer diverse populaties. Canada en Nederland beschikken
over een aantal internationaal goed aangeschreven patiëntencohorten met betrekking tot inflammatoire
aandoeningen. Het zou een flinke sprong voorwaarts zijn als deze cohorten worden samengebracht in een
netwerk waarbij een bijdrage wordt geleverd aan de ontwikkeling van gepersonaliseerde
farmacotherapeutische methoden voor de behandeling van IMIMD. Deze nieuwe vorm van samenwerking
tussen onderzoekers uit beide landen maakt het ook mogelijk om onderzoeksvragen te beantwoorden
over de toepassing van bepaalde behandelingen bij specifieke groepen patiënten om zo de zorg verder te
verbeteren. Ook levert het netwerk cruciale informatie opwaarmee beleidsmakers binnen de
gezondheidszorg het geld voor de gezondheidszorg uiteindelijk gerichter kunnen besteden.
Verder vormt het netwerk een uitstekende basis voor capaciteitsopbouw en biedt het veel mogelijkheden
voor het trainen van studenten en onderzoekers. Een sterke bijkomende factor van het netwerk is dat er
partners uit de publieke en particuliere sector bij betrokken zijn en tevens NGO’s, zowel uit Canada als uit
Nederland. Naast artsen, beleidsmakers en wetenschappers zullen ook patiënten betrokken zijn bij het
netwerk.

Twee fases
Het toekenningsproces van de financiering verloopt in twee fases:
In Fase 1 wordt bepaald welke cohorten het meest in aanmerking komen en worden publieke en private
partners aan elkaar gekoppeld, om te bereiken dat het netwerk heel Canada en Nederland beslaat en zo
representatief mogelijk van opzet is.
 Er is een procedure opgezet voor het indienen van een blijk van belangstelling (Expression of
Interest, EOI) om onderzoekers met expertise op het gebied van IMIMD ertoe aan te sporen om
innovatieve ideeën uit te wisselen over de manier waarop zij de genoemde doelen zouden willen
bereiken en hoe ze de sterke kanten van hun cohorten ten goede zouden kunnen laten komen
aan het netwerk.
 Voor aanvragers die belangstelling hebben om het netwerk mede aan te sturen, is een procedure
aan de hand van een intentieverklaring (Letter of Intent, LOI) opsteld. De intentieverklaring
moet gezamenlijk worden ingediend door een teamleider uit Canada en een teamleider uit
Nederland.
In Fase 2 worden de vier groepen aanvragers die naar aanleiding van Fase 1 een ontwikkelingstoelage
ontvingen, uitgenodigd om samen te gaan werken en een volledige aanvraag in te dienen.
Workshop voorjaar 2016
Vertegenwoordigers van iedere EOI en de mede-aanstuurders van het netwerk (de in Canada gevestigde
teamleider en de in Nederland gevestigde teamleider), geselecteerd op grond van de LOI, worden geacht
om deel te nemen aan een workshop van één dag, die in het voorjaar van 2016 zal plaatsvinden in
Toronto. Daar presenteren ze hun ideeën over de manier waarop zij hun rol willen vervullen binnen het
netwerk.
Deze workshop biedt diverse mogelijkheden voor het onderzoeken van mogelijke synergieën en
samenwerkingen tussen de verschillende cohorten. Het doel van de workshop is het ondersteunen van de
deelnemers bij het verder onderbouwen van hun aanvragen, zodat het netwerk op de juiste manier
tegemoetkomt aan de doelstellingen van de subsidieronde.

CIHR, the Netherlands Organisation for Health Research and Development (ZonMw) and the Dutch
Arthritis Foundation (Reumafonds) are pleased to announce a joint initiative: Canada-Netherlands
Personalized Medicine in Immune Mediated Inflammatory Musculoskeletal Disease Network, aimed at
developing personalized pharmacotherapy approaches to medicine for immune mediated inflammatory
musculoskeletal diseases through the research potential of multiple population-based patient cohorts in
Canada and the Netherlands.
Description
The treatment of a wide spectrum of immune mediated inflammatory musculoskeletal diseases (IMIMD)
has been revolutionized by the development of an expanding arsenal of targeted pharacotherapies. It is
internationally recognized that this pharmacotherapy revolution has come at a substantial price. Treatment
with any of the biologic therapies typically costs tens of thousands of dollars yearly for each person. There
is also an expanding spectrum of inflammatory diseases where these therapies are being used. In some
cases, this has resulted in unexpected outcomes, such as the recent demonstration that lethal forms of
vasculitis respond to depletion of B lymphocytes, which is an approved treatment for the treatment of
rheumatoid arthritis and lymphoma. The biomarker discovery revolution that has accompanied this
pharmacotherapy revolution holds great promise in personalizing the therapeutic approach to complex
inflammatory diseases.
A critical factor needed to support personalizing pharmacotherapeutic approaches to complex
inflammatory diseases is the need for large and well-characterized patient cohorts at the national and
international levels. Ideally, these cohorts should be organized into interactive Networks that can harness
the power from multiple diverse populations. Canada and the Netherlands have some of the world’s best
inflammatory disease cohorts. The ability to bring these cohorts together in a Network is an enormous step
forward.
Such a Network would seek to develop personalized pharmacotherapy approaches in immune mediated
inflammatory musculoskeletal disease. This novel collaboration of researchers in the two countries will also
allow for research questions to be answered regarding the use of particular therapies for certain patient
groups. Most importantly, the Network is expected to provide key information to ultimately help healthcare
policy decision makers to better target healthcare dollars.
Further, the Network is expected to be a great resource for capacity building, with ample opportunity for
training students and researchers. An added strength of the Network is that it will also include public and
private sector partners as well as non-governmental organizations in Canada and the Netherlands. It is
also expected that patients will be involved in the Network, alongside clinicians, policy decision maker’s
and scientists.

Highlights
The process through which funding will be allocated will have two phases:
Phase 1 aims to identify key cohorts and linking public and private partners in an effort to ensure panCanadian and Netherlands participations and representation.
 An Expression of Interest (EOI) process has been designed to encourage researchers with
expertise in the area of IMIMD to share innovative ideas on how they are proposing to tackle the
goals and how they will leverage their cohorts’ strengths to advance the Network.
 An Letter of Intent (LOI) process has been designed for those applicants who are interested in
being considered for the Network co-Lead roles. The LOI will need to be submitted jointly by one
Canada-based Team Leader and one Netherlands-based Team Leader.
Phase 2 aims to invite the four applicant groups who received a development grant in Phase 1 to
collaborate and submit a full application.
Strengthening Workshop
Representatives from each EOI and the Network co-Lead (e.g. Canada-based Team Leader and the
Netherlands-based Team Leader) of the LOI will be required to participate in a one-day workshop in the
spring 2016 in Toronto where they will present their ideas as to how they propose to fulfil their role as a
part of the Network.
This workshop will provide substantial opportunities for exploring potential synergies and collaborations
across cohorts. The purpose of this workshop is to assist applicants to strengthen their proposals in ways
that ensure that the Network adequately addresses the goals and objectives of the call.
